There module for shipping insurance option extra charge at check out?

 

charge option for admin:

Per item

total amount

I haven't seen one, but what we did was simply create a catagory called "shipping insurance" and then had products like $1-50, $51-100, etc. and then charged whatever the post office charges for insurance. It's worked fine so far - a bit kludgy but it works.

So at check out the customer able to chose if they want shippping insurance and which shipping method they want ?
