Product Options

I have a single product I want to sell, but I need to add a lot of options. The two options I need help with:

 

1) Some options allow the customer to write in information into multiple text boxes

2) The other option, I'd like to be check boxes (not dropdown menus).

 

How can I add these two options to the process? Any help would be greatly appreciated.. thanks
