Adding a custom form to a custom page?

I have made a custom pages for www.heidelbergflooring.com/shop , such as the shipping policy page. I need to add another custom page to "Request a Sample". I need it to have a name, address, city, state, billing information and wood type field. Once the form submits, it would simply email the results to the company and they would take care of filling the request.

 

Which page(s) would I need to add the custom form code?

Is there an easier way to do it than this, or a contribution that would make this easier for me?

I know basically 0 about PHP, so the easier the better.

 

Help fellas, I'm stuck..

well that sucks that you know 0 about php but you would need to create another page like the shipping.php file, and name it whatever you want, then you would need to go to catalog/includes/boxes/information.php and add it in there.

I know enough to use someone else's code and integrate it, but I couldn't hand code anything :blink:

I'll give your advice a try and see what I can some up with.

 

Sorry if my first post was unclear, I typed it in a hurry before I was leaving work.
