Create account page hangs

Desperately need help:

Background details: Running osc on windows local host - doing testing before I upload to a server. No SSL yet.

Problem 1: On create account page the state/province box is no longer a drop down - it changed to a simple text input box after I removed gender, DOB, company and suburb using admin/configuration/customer details. State remains "true".

Problem 2: Create account page hangs indefinetly even after all input errors are cleared

Help greatly appreciated
