How do I attributes to a product with this kind of form?

I have been trying to figure out how to add a form to a product. I tried using the product's attributes but it doesn't seem to let me add to the drop down menu or to add in a place for someone to type in quantities. Here is the URL of the form that I want to make for a product:

I need this form to be filled out and then added to the shopping cart with the price added up also depending on what a customer orders.

 

Can anyone help me figure this out?
