not all categories and sub categories are displaying

Isn't there some place where a limit on the number of categories is set? I've been adding products and now not all my categories/sub categories are displaying. They are still in the database. I think I've set it before but it's been so long I can't remember where it is.

 

Also, what makes the attributes display in any type of order? I was adding sizes and they were displaying in the order I added them and now they aren't. I'm sure I mucked something up somewhere but can't figure out where.
