Product attribute question

hello, my name is Matthew. I'm very new to the forums and hope to learn alot as time goes on. I wanted to ask you a question regarding something that I am wanting to be able to do, and I am hoping it will be an easy answer.

 

Here is what I need to be able to do on my stroe front, I will be selling custom items, in this case lats use an embroidered item as an example. Let's say the price for the item is $50.00 without personalzing the item. I would like to be have an attibute for the item so the customer can select to have the item personalized and it will automatically add a set amount to the price and then have a text box where they can enter the required perosnalizing information.

 

I hope I explained this correctly and maybe this is already available.

 

Any help would be greatly appreciated.
