Header Tags Controller - Product Entry Now Missing Name

I've just updated my site www.miva.com.au/store with Header Tags Controller V 2.6.1 and everything appears to be working well except for the fact that when I try to add a new product the form seems to be missing the "Product Name" field.

 

I tried going into the product and editing it but had the same problem there as well.

 

I've had a look through the notes and can't find if and where I went wrong and there doesnt appear to be any other topics in the forums on this. Anyone else had this issue or have a solution?

Actually, this problem comes up quite often in the Header Tags support thread. You have made a mistake in the categories.php changes.
