Where in OsCommerce are the definitions for a (hidden) text form input?

Somehow during some recent contribution installs, my website got messed up as far as text input forms are concerned. They used to be pre-populated with quantity data (as in the shopping cart), zip codes for shipping calculators, or the Credit Card holders name when you got to the payment page. Now all of a sudden whenever you view any of these fields you can type data into them and submit the data, but it won't show up anymore. (so for example if you change the quantity in the shopping cart it will update the price, but the quantity field does not show any numbers).

 

Does anyone know how or where the form functions are defined or where I could start to look to fix this? Is it something in application_top or somewhere like that?

They're located in the includes/functions/html_output.php file.
