I need a text field on product pages for my customers

I need a text field (Multiple Fields Would even be better) on product pages for my customer to enter custom info. We sell note pads and our customers have to enter the information that they want printed on there note pads (EX. From the desk of Bill). This information that is entered also has to show up on the invoice and the orders in the administrative section so we can print them.

 

If someone knows of a contrib. (Free is best) or how to add it manually I would appreciate if they could share their knowledge.

 

I do have an intermediate knowledge of php & sql - so I can be dangerous.
