Error at checkout_shipping.php. PLEASE Help!

We are suddenly getting an error at checkout_shipping.php. As of last week, everything was fine, and we haven't made any changes to the site...haven't even uploaded new products or anything.

 

Can anyone PLEASE point me in the right direction as to what has happened?

Could this be a result of some change my host made?
