Question about products adding in osc

I just installed SEO Ultimate URLs and maybe did some wrong thing and now i cannot add or edit existing products... it shows the page and showing me just the fields for product name and even seo url field and then page is blank not button to add or add more info for product i suppose its some restrictions maybe which i made wrong or don`t know ... if anyone is willing to help..

My site masksmarket .com and i will show whatever of .htaccess or even give access to someone if willing to help :P
