Missing shipping address city field in create_account2.php

I haven't been able to find any posts regarding my current dilemma - no city field in the shipping address entry form in the create account area in create_account2.php (I'm using Fast Easy Checkout).

 

Any suggestions as to where to start debugging this problem?
