I need to have a the following on my contact page

 

Address

Message Box (like the standard contact page)

and a tick box to subscribe to newsletters

 

how would i do this, is there a contribution which would work. i tried the contact us choice contribution but didn't like the look of it, and there isn't a newsletter facility..
