Problem with customer's address

The problem is in the address

When the customer makes the order his address doesn't show (but it exist in database, i know it)

This string show nothig:

<? echo tep_address_label($customer_id, $sendto, 1, ' ', '<br>'); ?>

 

what i must do?

how i can show the address???

The address_format table was empty and this post has the SQL to add to the DB
