Need Step By Step Instructions On Two Things

I need step by step instructions on two things:

 

1. How do I create a new box just like the ones I already have? My site was pre-built, so I have no clue how to do this.

 

2. How do I create an infobox that has script as it's output?

 

I want to incorporate/add the moon phases to my site, via ttp://www.calculatorcat.com/moon_phases/daily_moon_phases.phtml
