need help with making my own template

can someone give me some pointers on how to cut up the template and which parts should go in what files as far as header.php, footer.php, and index.pxp

 

can the shopping cart item be on the index.php file or does it have to be in the header.php file?

 

if i cant complete this would anyone be willing to help cut this up and implement this for me?
