Need Phone Number Field in Order Form

We run a gift basket business and it is imperative that we have phone numbers for both the person ordering the basket and a phone number for the delivery address. I am not fluent at all in php but can follow simple instructions as I have made other changes through the cpanel file manager. Can someone tell me in simple terms -- step by step -- how to add these two fields? Our site is http://adorablebasket.com.

 

Thanks in advance for any help you can give. We are running an older version of OSC ... osCommerce 2.2-MS2.

I also need fields for them to put their phone number and the shipping address phone number. But I don't want to screw up the site by doing something wrong.
