Parse Error in Age Verification contribution

I have followed the contribution for the age verification but I must have messed something up somewhere.

Now when you go to the registration page the birthdate option is now gone. Go into the admin section, glick on birthdate info, it is set to true. Clicking the update on this it gives me this error on the top of the page?

Can someone tell me what I did wrong? Or how can I fix it?
