Product image when updating information

Everytime i'm trying to update some information on a product, when i submit the update information the product image disappears.

I have to reupload the product image everytime i do some text information updates.

Is it because my installation is incorrect?

I just recently installed Categories and Products Images Folder Tree

and not sure if it's done correctly.
