Add a Tick Box to Checkout

Hey guys, i have a client that wants a tick box added into the checkout function.

 

They want it to say something like " Do you want to enter our competition" blah blah. With a tick box indicating "yes". On probably the checkout_payment.php page.

 

I have no idea how to add this , and then have it show on the order that gets sent to the owners order.

 

I looked at the "Agree to Terms" contribution , however if they dont accept that, it fails. So that isnt really suited for this, as they need to be able not to tick the yes option.

 

Any ideas?
