Let customer add info option

Hi All,

 

i have a store offering illustration services. What I'd like a customer to be able to do is upload a photograph and then add thier own description or comments about it. For instance, if they upload a photo of a home, they would then have a textbox that they could add the address.

 

Any ideas?
