Standard Credit Card Module

I have set up two other osCommerce sites with the same host that collect Credit Card payment information with the standard "Credit Card" module. With this particular site I am having an error when an order is posted. All other information is recorded and saved to an invoice, but I am missing the credit card data (below).

Credit Card Owner:

Credit Card Number:

Credit Card Expires:

 

All post blank in my invoice. However, the Addresses, Phone, Email, Payment Method, Products, Totals, and Date all all record fine.

 

Any ideas?

I'm having the same problem but with the Credit Card type and the CVV number.

 

Anyone any suggestions? Plus there's no field for these things on the checkout process.

I have two other osCommerce site running with the same host that are fine. I noticed that functional stores were running on IIS 5 and the broken store was running on an IIS 6 server. I had them move the store to an IIS 6 server and it seems to be running now.
