fatal error message with protx form on admin/modules

can anyone one help

 

i have just istalled protx_form-V1.0.9_1 and when i go into the admin > modules > payment modules i get the following error and cant do anything

 

Fatal error: Cannot redeclare class protx_form in /home/office2m/public_html/includes/modules/payment/protx_form.php on line 13

 

 

any help on hw to fix this
