When in doubt, disable 'Use Search Engine Safe URLs'

When you have these symptoms:

1. Shopping Cart won’t update – Can’t change quantity or remove items.
   
2. Password request won’t email you. No success message.
   
3. Tell-a-Friend displays “product not found” after you submit the email form. No success message.
   
4. Login Issues: Customers can create an account and stay logged in, but if they log off at some point, they can not log back in afterwards. Password will not work.
   

Do this:

1. Update related files & class files to current osC milestone (Can't be sure that this step actually helped, but I thought it was a PHP5 problem, and always a good idea to be current anyway).
   
2. Under Admin>Configuration>My Store, set 'Use Search Engine Safe URLs' to False
