Confirm Order Error

I admit - I am a bit out of my depth!

 

I have successfully integrated my CC processing gateway (eWay) and the CC transactions work when Confirm Order is clicked on the checkout_confirmation.php page however, for some reason, the result of this page takes me ultimately to domain.com which I assume is a default somewhere I have missed - but FOR THE LIFE OF ME - I cannot find this reference.

 

I know this is simple but help !!!
