Quick survery about where the customer heard about us? Any Contribs?

Hi all,

 

I'd like to add "Where have you heard about us?" type of a survey somewhere in the checkout process. It would be basically a drop-down box that lists few options. This would be great for finding out where the customer heard about us.

 

Any contribs that do this?
