survey anywhere mod-help needed with DB issue

Hi to all, i really need to make a survey up and running by next week for my products.

After exploring the options i noticed that there is only one conribution for making a survey through oscommerce: Survey anywhere

 

I installed it but when i try to view the survey page it gives me this error:

 

1146 - Table 'haircycl_os.TABLE_SURVEY' doesn't exist
