adding a field for information

Hello all, I am trying to set up a registration for a conference. I want to add a field where people can put in their license number and have a drop down that says "Do you need CE credits Yes or No"

 

I have been looking all over the site and found different things but now I am confused

 

Is there a simple or fairly simple way to do this?
