How to add a delivery calendar?

I was wondering how to add a delivery calendar so that the customer would be forced to select a specific delivery date (for a flower shop that does its own deliveries by car).

 

 

It does not need to be a graphic - it would be O.K. to just have a drop down menu with the dates listed there. Also, how do you make it so that the calendar would update with new dates automatically?
