Go to your Admin page and select Customer Details>Date Of Birth and change the value from True to False. That will eliminate that part. As for the phone number...I'm rather ashamed to admit that it's been so long since I disabled my own that I cannot remember how. Hopefully, someone will come along to help you with that one.
