Need some help Again :)

I installed a free shipping and also an insurance contribution. Im using the table rate shipping module.

 

Heres the problems

 

1: Shipping insurance isnt being carried over to paypal.

 

2:Need to remove free shipping from shopping cart on International Orders

 

If I setup free shipping on orders over X amount of dollars on Paypal everyone gets it. I only want free shipping on US orders.
