Layout requirement

Hi all ! :D

 

I am wondering if someone can help me with this. I would like to use the following product layout

 

 

but I can not seem to find out where I can change the layout such a way that the image (thumbnail) will be placed at top at the center, and the descriptions fields underneath in a order I can choose.

 

Also I had a look at a couple of contributions but need your help here also. Can someone tell me which contribution I can use to create the rounded graphical customized corners as displayed in the image ?
