Add "Dynamic" Fields to Product

I need to be able to collect some specific data from customers in order to process their purchase.

 

We are selling Season Ski Passes, as a Pack of 4. We obviously have the Purchasers name from the order, but that does not mean that the passes are all for them. We need to get the name and age of who each pass is for.

 

I would like to have the option to customize a name field, as well as age. Both required fields.

 

Is this possible, and how?
