Different Contact Forms for Different Options

I am having trouble figuring out how to setup different contact forms for different needs, I am familar on how to edit the code, but need an example on how to setup different contact forms. If someone can point me in the right direction, I'd appreciate it greatly. I need radio buttons for options and no mandatory text fields such as these forms below.

It has rodio select buttons for different emails such as admin, support, sales, so on. You can use this or modify it to your likings.

I needed a Text field like:

A drop down menu that would hold months of the year and would appear if they selected fundraiser option like:

 

JAN FEB MAR APR MAY JUN JUL AUG SEP OCT NOV DEC

 

 

or another drop down menu similar to above for wholesale for all the product catagories like:
