Custom Form..e-mails not beig recived, but no errors.

I'm trying to put in a custom form in my site.

here is the form's html and the sendmail.php file

I don't get any errors but I'm not getting the e-mail either.

The server is hosted by godaddy.com and the build-in oscommerce forms still get the mail sent. so I know it's not server side.(well 80% sure anyway.
