I have made changes where it shows the following and able to input the information. Full Name: Which was already there. Address: City: State: Zip: Country: Email Address: Telephone: Enquiry: Which was all ready there. But that is as far as I get. You can file out all this items, but when you send it a only the Full name - email address and enquiry are sent. How do I get the other add in to be sent. :'(
