Installed new theme - "My Store" module lists all fields 20 times now

I installed a new theme to a store that had been functioning properly. It's the OSCommerce template found here:

When it came time to install everything, I KNOW I put in all the server information for the database correctly - I cut and paste it from the hosting's admin. When I went to check, the theme was running properly. It can be viewed here:

Unfortunately, in the admin section, none of the store's previous settings are there and I don't know what database it's connecting to . But the worst thing is that under the MyStore module in the configuration section, every field is listed nearly 2 dozen times. I've tried deleting them and renaming them and they don't show up in the store's frontend.

 

What am I doing wrong? Does anyone know? Any and all help would be appreciated.
