Customized Item Atrribute/Write in

Before I set down and abuse the codebase any further trying to make this work, I'd like to see if anyone else has done this, attempted to do this, or released a contribution that my search-fu is too pathetic to find.

 

My client wants customers to be able to write in order items., whether it's for an attribute field for a personalization ("Happy Birthday Kimmy!") or for a full blown item. I had a working hack for this for full items that shoved things into the comments field but those proved unreliable for reasons I have yet to determine.

 

Does anyone have a contribution or even an attack vector for solving this problem before I go an mutilate the code any further?
