Easypopulate duping and placing items in wrong categories

I've finally got EP to work, but can't figure out why it's inserting each item 2x, first in the correct category, and second one into wrong product categories.

 

When I compared the import text file with the categories, it insert the first one in the correct category, and then the same item is inserted the second time in the wrong category using the one from item following it.

 

BTW the v_date_avail field is blank and the v_date_added is the date you see in the file.

 

Example:

 

1st insertion: .COM FOR MURDER Horror (correct category)

2nd insertion: .COM FOR MURDER Gamecube (dup and incorrect category, using the next item's category).

 

1st insertion: 007 (GAMECUBE) Gamecube (correct category)

2nd insertion: 007 (GAMECUBE) Xbox (dup and incorrect category, using the next item's category).

I know I'm asking for a lot, but I'm under a tight timetable to get this working. Any help/suggestions/hints will be greatly appreciated.
