conditional forms and individual order handling

Hello, I am looking for 2 mods:

 

1) custom, conditional forms: user fills in a form, where the next form page output is always dependent on the previous filled in form, and the end result can be added to cart as product.

 

2) individual order handling: after user A orders a product, admin gets the ability of adding additional individualised information to user A's order history.

 

Are there mods already existing for this?

 

Any kind of help is highly appreciated.
