Error messages installing ssl PROVIDER INFOBOX v1.0

I am getting the following error messages:

 

Warning: main(includes/ssl_provider.js.php): failed to open stream: No such file or directory in /home/agele3/public_html/index.php on line 92

 

Warning: main(): Failed opening 'includes/ssl_provider.js.php' for inclusion (include_path='.:/usr/lib/php:/usr/local/lib/php') in /home/agele3/public_html/index.php on line 92

 

 

Is this because my ssl_provider.js.php is in the wrong location?

Does anyone know how I can fix this?
