problem with "Credit Class & Gift Voucher"

I installed "Credit Class & Gift Voucher" (version 5.16 by BlueNoteMKVI Feb 8 2006)http://www.oscommerce.com/community/contributions,282/category,all/search,Credit+Class+and+Gift+Voucher on a new oscommerce site. Then i tested it by ordering some produts on my oscommerce site. But when after i clicked "confirm order", i got the below error:

What is the problem? This contribution is great. But because 1. the above error 2. it changed papal.php a lot, i am afraid of this, I do not want to use this contribution. Any contribution like this one i can use? Many thanks in advance.
