HTML help with payment form

Hello all

 

I am new to oscommerce and would like to make the following changes to the payment form:

 

-Add a drop down select box with credit card type ( Amex, master card or visa )

-Move the Credit card number field above the credit card owner

-Change the text from Credit card owner to Name on Card

-Add a Company field after the Name on card field and before the expiration date field

 

All change should be reflected in the database and any emails that are sent
