Please help - major login problem for existing customers

Can someone possibly have a look at my SQL file, i am having a major problem, which i thought would be fixable through a clean database install, but unfortunatly this has not been the case.

 

I have the following contributions installed:

 

Credit Class

Shipping Status

Remove page counter option in admin

Recover Cart Sales V2.11

Categories Box Bullet and line

Easy Populate

Wolfen Featured sets

Shop by price

phpBB integration contribution

 

Now i havent been able to fully test my store offline before uploading due to the apache problem with cookies (so if anyone knows how to fix this firstly, that would be great).

 

Now everything is working fine with my store, customers can sign up, checkout, post in the forum...alls good....except...

 

Once a customer has logged out, they cannot log back in. No error message just refreshes back to the login page? I have not turned off sessions ID's but none seem to be displaying, could this be the cause of the problem? Any ideas how i get them to work?

 

Here is my sql file if someone could kindly take a quick look to see if anything jumps out as an error?

Please note, this was a complete clean install with all my contibutions installed, taken from my offline store. I uploaded it into my database after dumping the original database, but still existing customers cannot login.
