Error At Checkout Confirmation

When I get to checkout confirmation, I'm receiving several errors. I noticed this a few days ago when I received payment for an order. I went into the database and did not find any order history, so I had to e-mail the customer.

 

Anyway, at the checkout confirmation page, I get this error both on the delivery address and billing address:

What do I need to do to rectify the situation? Thank you for the assistance...........................
