Adding forms within shopping cart

So here's what I need to do. I now have osCommerce set up to sell tickets for seating at my sound workshops. So for instance, a company might purchase 3 tickets for the Friday night workshop, 2 tickets for the Saturday workshop, and 4 tickets for combo Fri/Sat workshops. After these tickets have been selected and placed in the shopping cart, I then need to gather the names of the people these tickets will be assigned to. That is, I need to get information in order to print out name badges. So in this case, I want a form generated that requires 3 names to be filled in for the Friday tickets, 2 names for the Saturday tickets, and 4 names for the combo Fri/Sat tickets. And this form needs to qualify that at least something ends up in each required field. The information could end up in the Invoice itself, perhaps as text in the comments area.

 

Any way to do this you can think of? Does anyone here gather this sort of information on a per-item basis? Please point me in the correct direction.

There is a contribution that allows you to add text fields to your options/attributes and you could possibly do it that way but not sure if it would be 100% what you are looking for.

From the descriptions, both contributions have potential. Off the top of your head, do you know of any active websites using these contributions so I can get a look at how they work? If you don't know of any, I'll contact the writers and see if any sites are up and running.
