Trouble with Upload contribution

I just installed the files for the upload contribution so that I can have a customer upload a file onto my site. The problem that I am having is that when I go under product attributes in the admin tools. I get the following error message:

1146 - Table 'makeyour_p_os1.products_options_types' doesn't exist

select products_options_types_id, products_options_types_name from products_options_types where language_id='1' order by products_options_types_id

Because of this I am unable to put in any attributes. I have added the script in the configuration.php file. The only thing im not sure of is where the 3 .sql files go. Currently they are in my OsCommerce folder wit hthe mysql_catalog.sql file. Any help would be greatly appreciated.

I figured it out. But I am having a problem when I goto add the product to the cart. After choicing a file to upload and clicking add to cart I get an error message that says destination not writable. I have no clue how to fix this. Thank you for your time
