Adding new pages, problems

Hi guys,

 

I am trying to add a new page to OSC called cservices.php and I have followed this thread exactly -

However I get the following error when I try to view the page -

Any ideas on what Im doing wrong. Any help apprciated.

Have you added your new page into the filenames folder.
