Restore orders/cancel order contribution help

In regarding this following contribution

The contribution itself seems to work fine. When I go to the Cancel_Order.php page the page drops the boxes, the top left and right of the header. It just doest look right and I would like some help on how to fix this if possible. It just looks buggy when some of the pages formatting is missing.
